Jackie Griffin, RN, BSN is a Care Coordinator for Tahoe Forest Hospital. She is a graduate of the University of Maryland School of Nursing and came to this position after working for many years on the Medical- Surgical Unit at Tahoe Forest Hospital. Her diverse nursing experience includes: In-patient Rehabilitation Admissions and Education Specialist, Orthopedic, Telemetry, Neurological and Geriatric Nursing experience as well as Director of Nursing in Long Term Care.
As Care Coordinator, Jackie works with people in the community who live with chronic illness. She assists them with navigation of the health care system, improving access to primary care practitioners, community resources and specialists. Care Coordination is a patient- and family-centered, team-based model of care designed to promote self management and improved quality of life.
